Digital copies of your records can easily be requested by filling out a release form on this site—just click the "Request My Records" button to get started. There is a fee for copying and delivering records, which helps cover the cost of securely and efficiently fulfilling requests. All charges are in full compliance with applicable state and federal regulations.

Since 1990, Crozer Health has proudly served residents and families throughout Delaware County. Through our hospitals, ambulatory surgery centers, clinics, and doctors’ offices, we’ve had the privilege of providing comprehensive, quality inpatient and outpatient medical services. We’ve also maintained a longstanding commitment to training new generations of physicians and other health care providers, while also supporting programs that improve the health and well-being of the communities we serve.
Given this history and the role, we’ve been fortunate to play in your lives, it is with a heavy heart that we share that we’ve made the extremely difficult decision to begin winding down operations across our Crozer Health facilities.
As of April 30th, 2025, our hospital’s emergency room will no longer be seeing new patients. In addition, our physician offices will be closed to all patient visits after close of business on Friday, May 2nd, 2025.
